Additional Information Department: Cardiology Hours per week: 40, eligible for benefits Shift: Monday- Friday days Job Overview The Clinical Nurse Care Coordinator is responsible for facilitating and managing care coordination of designated populations of the most complex pediatric patients within an ambulatory environment. Leads internal and external care team partners in the delivery of high-quality, safe, personalized, and cost-effective, outcomes-based, family-centered care. Responsibilities POPULATION SPECIFIC CARE - Neonate - 30 days to 1yr - Toddlers - > 1yr to 3yrs - Pre-Schoolers - > 3yrs to 5yrs - School age - > 5yrs to 13yrs - Adolescent - > 13yrs to 18yrs - Adult - > 18yrs to 65yrs ESSENTIAL FUNCTIONS An employee in this position may be called upon to do any or all of the following essential functions. These examples do not include all of the functions which the employee may be expected to perform. 1. Collaborates with patients, families, and providers to establish an effective plan of care based on mutually developed goals and in an environment that fosters shared decision making. Specifically as it relates to coordination with the Maternal Fetal Care Unit, NICU, and CICU. Arranging for cardiac surgeon/ cardiac interventionist meetings with family as appropriate. Arranging delivery planning meetings, and touring new families through the MFCU, NICU, CICU as appropriate. 2. Operate as the clinic facilitator, making sure patient history and pertinent new information is collected and shared with the team at each visit. This includes reviewing patient charts at the beginning of the day and sharing history information/ indication with the cardiologist and the sonographer. 3. Assesses patient's and family's unmet health and social needs in the hospital, at home, or in other community locations. This involves coordinating with social work and the MFCU psychologist, and making referrals to the SIE center, 22q11 deletion clinic, Inherited arrhythmia clinic, etc. Making follow up phone calls after a new diagnosis, answering questions, and offering continued support. 4. Monitors patients' adherence to plan of care and progress toward goals. Facilitates and documents changes and/or interventions as needed, including making sure proper follow up is coordinated with local and regional maternal fetal medicine and cardiologist specialists. 5. Provides education and coaching to patients and families to improve health literacy and work towards self-management of their health. Being responsible for all the specific educational handouts, assembling new diagnosis information including most recent outcomes and transparency information. 6. Creates ongoing processes for patients/families to determine and request the level of care coordination support they desire over time. 7. Facilitates patient access to appropriate medical and specialty providers as well as other care coordination support, including, but not limited to, medication acquisition and management, procedures/tests, appointments, and referrals. This includes arranging for out of state patients to be seen at CHCO, and relocate to the Denver area as needed. 8. Serve as a point of contact, advocate, navigator, and informational resource for patient, family, care team, payers, and community resources. Facilitates related meetings as needed, including every Tuesday morning at the Colorado Fetal Care Center weekly meeting. Making sure the list of new cardiac patients is updated and shared with the team as appropriate, and reviewing the status of patients in the CICU and CPCU with the team. Attending the twice monthly Fetal Hearts meeting, as well as the Monday huddles. 9. Provides mentoring/coaching to other population health and care coordination team members. Works very closely with the other nurse coordinators in the CFCC. 10. Participates in practice transformation efforts that support ongoing coordination of care for medically complex patients and families through sharing knowledge, acting as role models, and providing technical assistance. Following up with patients after birth, and continuing to support the needs of the family. This may include attending deliveries that occur during the day, and facilitating warm handoff to the CICU/ NICU teams regarding plan and what has been communicated with the family. 11. Participates in the development and revision of policies and procedures, as well as with quality improvement and research activities impacting care for designated patient populations. Works closely with the research coordinator to identify potential study patients, and notify the team of the patient study participation. Maintain a fetal cardiac Redcap database, run reports, and report number of fetal echos per location monthly. Send weekly list to appropriate team members notifying of upcoming deliveries for inpatient team staffing preparation. 12. Utilizes information available within the organization in conjunction with technology to support communication, clinical decision making, error prevention, and overall care coordination. 13. Works with cardiologist, fetal team, and marketing to develop and implement strategies to expand our services. Serves as an ambassador at Lunch and Learns at OB, family practice, and midwifery practices, as well as lectures and tele-echo outreach events. Other Information SCOPE & LEVEL - Guidelines: Full performance professional level work that requires additional knowledge, education, training or experience. - Complexity: Duties performed involve concepts, theories, and concrete factors to be evaluated and weighed requiring a high degree of analytical ability, independent judgment, and decision-making. Acts as a subject matter expert in the field of expertise. - Decision Making: Exercises judgment and discretion, and is responsible for determining the time, place and sequence of the work performed. - Communications: Contacts are of a remedial nature involving the resolution of problems and where some degree of discretion and judgment are required in carrying out a major program and/or function of the organization. - Supervision Received: Performs work with minimal supervisory oversight. Work assignment is generally unstructured and employee is responsible for organizing complex, varied, and simultaneous coordination of several functions, programs, or projects in various stages of completion. Qualifications - Education: Bachelor's degree in nursing (BSN) is required. - Experience: Minimum of four (4) years of nursing, case management or care coordination experience in a pediatric environment. - Certification(s): - BLS/CPR from the American Heart Association with at least 6 months left before expiration is required upon hire. - Achievement and maintenance of Care Coordination and Transition Management (CCTM) certification within six (6) months of hire or transition into role. - Successful completion of internal Care Coordination training within six (6) months of hire or transition into role. - Licensure(s): Registered Nurse (RN) licensure in the state of Colorado is required. - Professional Membership(s): Active membership in a professional nursing or healthcare organization is required. Physical Requirements - Body Movement: Will frequently stand/walk up to 12 hours per day. - Body Movement: Will frequently use hands while keyboarding and writing. - Body Movement: Will frequently sit up to 2-4 hours per day while accessing computer, talking on the telephone, and while attending meetings. - Back and Upper Body (lifting and carrying): Will occasionally lift/force exerted up to 100 pounds. - Vision: Constantly uses sight in performance for daily job duties. - Hearing: Constantly uses hearing in performance for daily job duties. - Others: Able to assist or assume direct patient care as census directly permits. Exposure to unsteady or falling patient: In the course of transferring a patient, may encounter a patient who falls or loses balance, causing employee to fall and/or bear the full weight of a patient.
